Healthy Siouxland: Improving the Health of the Community

Siouxland District Health Department and a variety of community partners are working to improve access to physical activity and healthy foods and beverages through a variety of initiatives.

Siouxland Community Health Needs Assessment

Siouxland District Health Department, along with Dunes Surgical Hospital, MercyOne and UnityPoint Health – St. Luke’s complete a Community Health Needs Assessment (CHNA) and Health Improvement Plan (HIP) every three years.

The purpose of community health needs assessments is to learn about the community: the health of the population, factors that lead to higher health risks or poorer health outcomes of certain groups, and community resources available to improve the health of the community.  The purpose of the health improvement plan is to describe how the community will work together to improve the health of the population.  The community health improvement plan sets priorities, directs the use of resources, and is a roadmap for the development and implementation of projects, programs, and policies.

The joint CHNA looks at data from the following six counties in the tri-state Siouxland area, as each hospital serves a larger territory outside of Woodbury County:

Union, SD

Dakota, Dixon and Thurston, NE

Plymouth and Woodbury, IA

The top health issues identified for 2019 – 2021 are:

  • Mental Health
  • Homelessness/Housing
  • Obesity/Chronic Disease
  • Sexual Health
  • Substance Abuse
  • Access to Healthcare Services

About Healthy Siouxland

Healthy Siouxland is a coalition designed to improve the health and wellness of residents in the Siouxland area. We are a cross sector group of agencies that meet on a quarterly basis to share project updates of several working coalitions and grants that are related to chronic diseases, physical activity and nutrition.
